Below is a list of the information required at sign-ups including dates and locations for you to consider:



REQUIRED REGISTRATION INFORMATION:

1. Copy of Wrestler's birth certificate (NOTE: If your wrestler received an IKWF card during the 2008-2009 season, a birth certificate is not required for the 2009-2010 season registration.)

2. Completed Jr. Dukes Registration Form (click for document)

3. Completed IKWF Application (click for document)

4. A check for your registration fees (checks payable to Elmhurst Jr. Dukes Wrestling)

SIGN-UP OPTIONS:

A.  Sign up at a registration table at Helmut Berens Park football fields on:
             *  October 6th, 2009 (Tuesday)   6pm- 7pm
             *  October 15th, 2009 (Thursday)   6pm- 7pm

B.  Sign up at a registration table at York High School (door #5) on:
              *  October 22nd, 2009 (Thursday)  6pm- 7:30pm
              *  October 27th, 2009 (Tuesday)  6pm - 7:30pm
              *  November 9th, 2009 (Monday) 6:30 - 8:00pm

C.  Sign ups are on-going, visit the next Jr. Dukes Practice and sign-up for the 2009-2010 season!
